Here's my list:
- the sign of starry pawprints sent to Leafpool to convince Squirrelflight that she was right to get back together with Brambleclaw. So lazy in terms of character development.
- the sign of walls of bramble to convince Firestar to choose Brambleclaw as his deputy. Same, very lazy. Especially since as a reader and given Brambleclaw's actions at that time (he was in besties mode with Tigerstar and Hawkfrost), it's hard to understand why StarClan loves this guy so much??
- the tree that felled Mudclaw. If you can do that, where were you during Brokenstar and Tigerstar's reigns?! It would have been better if other cats had been stuck under the tree with Mudclaw.
- StarClan's intervention at the end of "The First Battle". Let the living have free will and make their own decisions!
- Tigerstar II's resurrection. When ancestors who are supposed not to choose the leaders of the living defy death itself to bring back their favorite.
- The trials, obviously. They should have been established AFTER "The Broken Code", so as not to contradict Bluestar's explanations in Arc 4 and StarClan would not become completely incoherent. Plus, it would have made Ashfur's impact on the Clans even stronger.
What do you think?

when they wouldn't make it clear who the fourth cat was it actually annoyed me so hard. not the time to be mysterious guys!! also, deciding to take in ashfur and onestar and countless other morally questionable cats, but having to have a whole trial for leafpool, almost not letting her in because she did the thing they literally told her to do.

The Ashfur and Juniperclaw afterlife mistake. Both were sent to the wrong afterlife and while it is somewhat fixed now it still was a huge mistake on StarClan's part that almost destroyed the clans.

Not because of Starclan themselves but I hated “Fire alone will save the clan” because Fireheart is such a bumbling idiot.

The sign sent to Cinderpelt in Midnight about a tiger leaping through flames, signaling the pair's importance in discoveing the Lake Territories. That determined Firestar to separate Squirrelpaw and Brambleclaw because he misinterpreted the sign, thinking their relationship would prove out to be toxic for both their Clan and themselves. Considering no other cat except for the 4 of each Clan knew about the prophecy of "Air, Water, Shadow and Sky" at that time, I don't see any need in alerting cats who would end up misinterpreting this omen because they didn't know about the coming Sun-Drown Quest.

always torn on the reeds in ravenwing's den. there's a lot of ways to interpret it that could be very interesting but the entirety of mv is so short they don't really go into it. but also I feel like that kind of adds to the "wait wtf" of it all, but mv still probably could've done better as a longer book, but at the same time ... etc

- Any of the Brambleclaw signs. The fire and tiger prophesy was super unclear, the starry pawprints to convince Leafpool that Squirrelflight was right to get together with him, which was super lazy. (Especially ironic considering how bad their relationship is) and the brambles protecting the camp, also super lazy, and frustrating since there were probably better options for deputy anyways.
- Choosing the leaders: Mudclaw and Tigerstar II. Mudclaw was shoved out of the leadership by StarClan's actions, and Tigerstar II had the leadership pretty much handed in to him. (Granted, he was the last deputy) I think it would be more interesting if StarClan was acknowledged to change and be biased. Maybe they overcorrected for letting Tigerstar 1 survive? But we do not see this at all.
- Many of the signs in AVoS. It just felt like it was pulling the plot along. I was happy that StarClan was gone the next arc.
- Afterlife mistakes, yet still keeping the trial system. How did Ashfur pass and Juniperclaw didn't? This is never really rectified.
- Whatever they did to Squirrelflight. Why did they lie to her to have her take the kits, then put her under trial for doing exactly what they wanted? It both denies her usual agency, and is super hypocritical.

-Squirrelflight’s Hope What they did to Squirrelflight and Leafpool was so incompetent and dumb. Yellowfang told Squirrelflight to take in the Three, then they both got social consequences from the lie during OotS and were forgiven for it. So revisiting it felt redundant and unnecessary. During the trial, they say that StarClan could have chosen any Three for the prophecy, but we know that’s not true! The prophecy came from beyond StarClan and they were even surprised to learn Hollyleaf wasn’t one of the Three! Every time I think about this, I get the urge to go on a rant on how dumb it is
-Splitting up in Omen of the Stars In concept, the idea that Jayfeather gets to use his powers to unite the afterlife sounds really cool. But StarClan splitting up made them look so incompetent
-Blaming Spottedleaf for who she fell in love with ??? What
-reincarnating Cinderpelt. As if she did not have a good life being a medicine cat. As if other cats weren’t robbed of opportunities for reasons beyond their control.
-Whatever was going on with Goosefeather (he wouldn’t be alive for half the prophecies they told him!)
-Insisting they make no mistakes in TBC… an arc where a StarClan cat went evil. Insisting that redemption is impossible for Dark Forest cats, right after some of them helped save StarClan. Juniperclaw even died thinking he would be redeemed. This feels dystopian and cruel. I wouldn’t want to go to StarClan after hearing this! I thought arc 8 was going to be more of a response to this.

The fact that StarClan will let the Clans suffer for the sake of being vague. A few cases in point:
-Beware a warrior you cannot trust, the evil force: no, we don't need to tell ThunderClan that Tigerstar's luring vicious dogs to them, just send (sorta) vague messages to Bluestar and Fireheart. Swiftpaw died and Brightpaw got maimed? Oopsies! Brindleface was killed to give the dogs a taste for cat blood? Oh no, anyways... Oh, and Fireheart almost got mauled and Bluestar drowned to save ThunderClan *shrugs* (This one's weird in general because given Fireheart's history with Tigerstar and the evidence of dogs, it should have been obvious, but they made Fireheart stupid for plot so...)
-The dying warrior: no, don't tell the Clans where to go, just let them argue amongst each other, suffer, starve, deal with trauma from watching their homes and camps get destroyed, get captured, and die out while they're waiting for a shooting star. Because the situation's not dire enough that they can't wait for a shooting star.
-The fourth cat: couple other people explained the problems with this one above.
Listen, I know this is all for plot's sake and StarClan can't tell the Clans everything and yada yada so it's pointless to complain, but god does it irritate me in dire situations like these. Plot doesn't change how ridiculous it all sounds when you really think about it.

-reincarnating Cinderpelt. As if she did not have a good life being a medicine cat. As if other cats weren’t robbed of opportunities for reasons beyond their control. How could I forget to mention Cinderpelt in my initial post, when it's one of the decisions of the authors that I HATE the most...
I absolutely do not understand this choice. As you said, how did Cinderpelt have a more miserable life than all the apprentices/kittens who died very young? How did she have a more atrocious life than 80% of the characters in this universe?
If it's because she never got to get together with Firestar, then Spottedleaf should be reincarnated as well? If it's because she suffered emotionally, then Bluestar, Yellowfang and so many others should also be reincarnated? If it's because of her injury and disability, then absolutely ALL the characters in this situation should be able to be reincarnated (Deadfoot, Jayfeather, Briarlight, Lilywhisker, Brightheart, Snowkit)? And it pisses me off even more because Cinderheart is so BLANK compared to Cinderpelt. She doesn't even have a real personality (what makes her different from her sisters, really?). Her story in Arcs 3 and 4 are so boring and poorly executed. To conclude, it completely ruined Cinderpelt's death in my eyes, which I had loved when I read it the first time. The impact is much less, knowing that she didn't really die.
