We have seen in the series disputes sometimes be resolved in something like champion warfare, where each side sends forth their most impressive warrior, they duel, and the duel decides the outcome without need for a larger battle. Why don't more cats resolve things like how Mudfur and Adderfang did?

Wasn't Tigerclaw considered something like that by ThunderClan in the first arc? Although certainly not to the extent it was outright mentioned more specifically with Mudfur and Adderfang. And since those two examples happened in additional books taking place before Tigerclaw's "time to shine" (he was already alive in terms of Mudfur at this point though) but got written after the first, second and even third arc, might have something to with the concept of Clan champions not getting explored much further than in SEs/Novellas.
For example, Lionblaze would've been the perfect choice as a champion for ThunderClan during PoT/OotS when he had his powers and was sometimes sent (or went himself) after dangerous individuals (that one fox and Breezepelt for example).
Other than this, there was also Hawkwing from SkyClan going one on one with Dodge and killing him. Which decided the fate of Dodge's group who had taken a few of the SkyClan cats captive at that time (again only happened in an SE). Granted, this was not really about Hawkwing being considered SkyClan's champion (he was just trying to save his apprentice and Clan) although he definitely has the potential, in my opinion.

It seems like maybe this is frowned upon, considering RiverClan's reaction to Mudfur. Clans are all about kinship, teamwork, and fighting together, so sending out one champion to fight alone for the entire Clan isn't something they do commonly.
But it is cool, and I would like to see more of it.

Clans are all about kinship, teamwork, and fighting together, so sending out one champion to fight alone for the entire Clan isn't something they do commonly. But it is cool, and I would like to see more of it. That's a fair point, if the champion combat decides everything, then it denies all the other warriors their own chance to win merit on the field, so there's a certain lack of incentive there. Still, at least with how warfare between the clans has become so ritualized and bloodless lately, it would seem that champion combat would be in a good thing to return to nowadays.
