The entire issue with SkyClan's territory in AVoS. Yes, there was more forest behind the Clans' territories, and apparently no one thought to explore it because it was too "wild"
So instead of anyone jumping on this simple fix to spread their territories further away from the lake, we get nice, juicy long-winded drama in the form of SqH

Besides what’s been mentioned so far , I’d say the novellas Leafpools wish and Dovewings Silence, especially Dovewings . I mean I don’t understand the logic of releasing a Book that was intended to give better closure, and some pretty good character development for Dovewing, seriously one of the few times I actually liked her. only to have it all thrown aside immediately after with Bramblestars Storm .

The Clans somehow twisting the words "you can have it when we move" into "let's take it right now" despite the fact that a. Leafstar wanted them to move first before SkyClan moved in, b. the Sisters were there first, c. it was never their right to dictate whether they should have the land or not. At all.

Rock blaming Jayfeather, Lionblaze and Dovewing for the rise of the Dark Forest. Like, what the **** was that? Also all the clans freaking out in Dark River because the WindClan cats were practicing hunting something else than rabbits, that was also a beautiful wtf moment.

Nobody realizing Mistyfoot and Stonefur were Bluestar’s missing kits is one of the biggest plot holes in the series that just gets bigger the more it’s mentioned in the books. Misty and Stone kept their ThunderClan names. In Leopardstar’s SE, all of RiverClan knows Graypool’s litter of kits has died, and since medicine cats assist in giving birth, Mudfur would have known for sure that Misty and Stone were not true RiverClan born. Not to mention, in the original arc, where Fireheart mentions Misty and Stone apparently look uncannily similar to Bluestar multiple times.

I think the Leafpool/Squirrelflight situation is worse than this. The Bluestar situation can at least be somewhat explained by how most cats don't really pay attention to the new kits until they become apprentices and tend to not have the best memories when it's not backed up by smell (like when no one recognizes Graystripe when he returns and doesn't smell like ThunderClan), though they still should have changed the names. On the other hand, a whole arc relying on the premise that no one who lived with a cat throughout their pregnancy realized that said cat was pregnant, or that another cat was not pregnant, not to mention them giving no reason for the unprecedented and dangerous act of giving birth outside of camp in the snow, is just unforgivable.
