It's never directly stated, but when we see her in StarClan, she's clearly able to get on just fine, so this implies that it was in fact healed after she died.

No, she didn't keep her injury, I believe. I'm guessing Crookedstar kept his injury because it was not only his namesake, but minor damage that wouldn't completely change the way he functions. Of course, I believe Deadfoot kept his too, which means that he walks with a limp...yea, StarClan is confusing. Then again, both Crookedstar and Deadfoot's weren't enough to hold them back at all in any way shape or form, and Deadfoot's didn't exactly hurt him.
Since Longtail, Cinderpelt, Jayfeather, and Briarlight would be hindered a lot, with them having their functions changed completely by their disabilities, I suppose it makes sense that they would lose them. It is implied that a cat is in perfect condition and health in StarClan, so Deadfoot and Crookedstar keeping theirs makes sense since it doesn't change that part of them. Deadfoot got on just fine whereas Cinderpelt struggled and likely felt pain if she moved her leg wrong. StarClan also alleviates pain.
Makes sense for a spirit to not keep their old injuries/disabilities/deformities that they had in their body and yet choose to present minor ones visually if they were a part of who they are. Note that Deadfoot and Crookedstar both were named after theirs. And Brokenstar, though I dunno how injuries work in the Dark Forest.

I haven't gotten that far in the series yet (or I don't remember it very well), but I doubt it. I believe it was implied if not directly said that cats who join StarClan are in the forms of when they were the happiest. Yellowfang remained an elder because her happiest and most fulfilling moments of her were when she joined ThunderClan, for instance, if I'm remembering that right. Now here's an interesting side branch. Imagine Cinderpelt coming face to face with Cinderheart in StarClan.

I think I heard that in StarClan you can choose if you want to keep your injuries. I guess Cinderpelt chose to get her leg healed, so she could hunt again.
