I'm not sure what you mean exactly but it does seem quite popular at the moment to draw various characters (especially medicine cats or cats with plant names) with leaves in their pelt. In cannon this doesn't have any basis from what I remember and the only time cats have been shown with leaves on their pelts have been as part of some kind of medical treatment. That being said cannon isn't everything and I think it's a nice way to differentiate between cats in fan works and looks quite nice, especially as a visual way to denote medicine cat status (like how some people use stars for leaders).

It's just character fanon design. Especially for Med Cats. (For me I also do it for Tree because he is hippy dad to me)

I've never seen that particular design before- this is my first time hearing of it. I've only seen the "leaves behind ear" thing.
