Remember, this is from the perspective as you as a hypothetical leader whose word can now shape the warrior code forever (or until it's challenged or added onto again), not as a fanfic writer. Although, perhaps that too... Hmm... Anyway, yes, you're leader of one of the 5 Clans. What additional rule would you personally add to the current 16 present within the code, and why would you choose to add that particular law? Maybe this could lead to some discussions!

If I’d be able to add one rule to the warrior code it would probably be that no kit/apprentice could be exiled.

Kits/apprentices can choose what Clan they can live in but once they made the choice, they must stay loyal to that Clan.

Can it be to make Permaqueens a regular and respected role? In all clans? I think it's more important than the mediator role

Honestly, I'd like a grace period rule. Apprentices should be given a grace period to which they can choose their path and what they want to do toward their future. Whether that's being a medicine cat, a warrior, a mediator, a permaqueen, or even possibly switching clans, whatever it is, I think it should be decided before they make oaths to their clan. I think oaths are pointless if they're going to pledge to them then turn around and break them constantly. They should take responsibility for their actions and live with their choices. When a cat knows that they are willing to stand with their clan and serve under their said position, then they should be given their full name. Their names would have more weight and meaning to it, because it's something special given to them from their clan and it's also earned through their loyalty and dedication.

I want forbidden relationships to be allowed. This rule existing alone causes so many problems. It ruins relationships, hurts other cats, there's a lot of lying, pain, and many other problems because of the rule. I feel like a cat should be allowed to fall in love with someone from a different clan if they want to. It shouldn't be treated as punishment. Look at Leafpool: she lost everything, and her own daughter even wanted her dead because she fell in love with Crowfeather. Her entire life fell apart after the truth came out. Even though she made the right choice and returned to her clan, she still gets shit from StarClan about it.
Forbidden love should be allowed and I feel like this should only be a rule for medicine cats. I understand why it should be different for them, however, it could also be argued that every cat is different and Leafpool alone has proven that she doesn't pick her kits over her clan. But for regular warriors? Let this rule be broken. If found out, a cat's punishment should only be the leader making them choose if they want to be with their lover or their clan. No exile; no harsh treatment. I'm so tired of everyone in the books getting treated like they're some horrible monster for breaking the code. And a cat should be allowed to visit their kin or sibling if they choose to pick the lover's clan over theirs without shame. There is nothing wrong wanting to visit the cats you care about.

In general, I think making the Clan cats less rigid-thinking would be a good start.

In the event that the Warrior Code directs a warrior to take two contradictory actions, the warrior cannot be punished for breaking one of the Codes if they were following the other.
