would have happened if Puddleshine wasn't able to heal and died from his infection?
My theory: ShadowClan would have blamed ThunderClan for not trying hard enough and/or purposely letting him die. I think Tigerstar would have wanted to start a war(while still disputing with SkyClan), but someone points out that they don't have a medicine cat and if a clanmate got severely hurt in battle they wouldn't have the knowledge to heal thier poor dying clanmate. Tigerstar then demands that ThunderClan lends one of their medicine cats(again!) to help ShadowClan. And possibly start training Shadowkit early.
I don't have anything else to add to this /shrug

I was thinking that Tigerheartstar would blame ThunderClan, especially Alderheart. He'd probably also reference Flametail's death to grind Jayfeather's gears like he does in the canon story.

Once again, Leafpool would head over to ShadowClan and try to teach Shadowkit the medicine cat ropes. There would be more tenseness between ShadowClan and ThunderClan.

Tigerstar would make Alderheart punishment be stay in Shadowclan to train Shadowkit probably, and because that apprentice is the leader son I can totally see Tigerstar wanting the complete package instead of the rushed training that Puddleshine had.
Alderheart would accept by guilt, to not cause war, sense of duty and let's be honest, he loved having Shadowkit as an apprentice too.

Alderheart would accept by guilt, to not cause war, sense of duty and let's be honest, he loved having Shadowkit as an apprentice too. Yeah, I can see Alderheart staying way longer than Leafpool did with training Puddleshine. Either Tigerheart would insist on it (like you said, it'd probably be related to favoritism) or Alderheart would feel bad and want to remedy it, so he offers to stay himself as a sort of self-inflicted punishment. It's certainly a plus that he became so fond of Shadowkit, but being forced to stay there for god-knows-how many moons -- don't medicine cats take far longer to train than warriors, who usually get their name at twelve moons after training for six? -- would be a kicker on anyone, but especially a cat like Alderheart, who'd be carrying the guilt of Puddleshine's death. He'd also definitely be faced with tons of hostility from ShadowClan and cold, snide remarks behind his back and to his face, since they'd see him as the cat who murdered their only medicine cat. Even if Puddleshine visited somehow, like Tree contacting him, to insist it wasn't Alderheart's fault, I imagine that wouldn't change much...especially with current ShadowClan.

1. Alderheart’s boyfriend would be dead 2. Tigerheartburnstar would punish him by holding him captive 3. Having Shadowkit become an apprentice early so Alderheart could train him 4. Hopefully Dovewing notices what an ass Tigerheartburnstar is being and divorces him and she’ll slowly come back to my list of favourites
