Drop them as a friend.
Yesterday I was called and I quote "a tall adorable giant" by one of my new male high school friends and heck I'd believe a positive comment more than I'd believe a negative one. Also if they're just going to call me ugly out of the blue then I'd just roll my eyes and say, "Not sure why it was necessary for you to say that, but I guess you need to get yourself sorted out if you're suddenly judging people in such a shallow way."

Be extremely upset, worry about it incessantly and try to work out what they found ugly so I could change it or fix it and hopefully improve their opinion of me.

Depends on context. My friends and I usually call each other names in good fun but if it was a serious situation and they actually meant it they wouldnt be my friend anymore.

probably be pretty damn upset about it if it was an actual statement, and not a joke but jokes?? i think i have at least 2 friendships built on insulting them and they insult me back jokingly, literally all my friends get the affectionate term of "dumba**"
