I can't help but feel Ravenwing's omen was misinterpreted. Concidering the circumstances during the time, it makes sense to think it would be about Mapleshade's kits. But what if the omen actually represented Jayfeather, Lionblaze, and Hollyleaf?
I know it seems a bit strange that StarClan would send an omen about three cats who wouldn't be born for several more years, but this isn't the first time they have done something like this. All throughout Moth Flight's Vision, she sees glimpses of the future cats. And in Bluestar's Prophecy, they send Goosefeather an omen that actually represents the battle in Eclipse. And again in Firestar's Quest, Firestar is told about the birth of the Three long before it happens. There are several more omens throughout the books that hint to future events. So it does not seem tol far fetched that StarClan would pull something like this off.
But why would StarClan use water and reeds to represent the three? For sure, that would seem like it clearly represents RiverClan, right? I thought so too at first, until I went back and looked at Cats of the Clans. On Squirrelflight's page, it says this: "If Leafpool is like water, calm, deep running, reflecting the stars, then Squirrelflight is like fire." Leafpool has also been described as a medicine cat with a destiny unlike any other.
Ravenwing's omen is described in a very similar way. "A tiny stream appeared in my den, in a place where no stream has run before. It carried three pieces of water reed with it."
Streams are very calm and gentle, just like Leafpool. The stream headed into the medicine cat den, because Leafpool was always meant to be a medicine cat, with a destiny unlike any other. That's why the stream is so out of place. And since she is a medicine cat, her kits are represented as plants in the omen. The three also have connections to medicine cats outside of Leafpool.
This is probably a stupid theory, but I figured I would share it anyways.

Your theory is about as good as any I've seen. I really hope StarClan didn't decide to bring down the hammer on Mapleshade.

Ravenwing was a jerk. I am fairly certain he lied about the omen to stir up trouble, since he learned about Mapleshade's kits. He thought it was wrong, and told her so. And then he suddenly has an "omen"? Yeah, okay. No medicine cat would ever put kits in danger--but Ravenwing did.
