In the first series it seemed to imply that most house cats got taken to "the cutter". The clan cats were well aware of this and likely looked down on kittypets in some way because of this (at least that's how I saw it). What always annoyed me as the series went on was that kittypets seemed to never be de-sexed. Prime examples are Millie and Greystripe. Greystripe would have likely gone through a shelter and you'd think they would neuter him in order to prevent unwanted kittens from being born. Millie has likely been a kittypet her whole life and I expected her to have been to the cutter at a young age.
What do you guys think about this? Any reason you think this happened?

Not all pets get neutered/spayed. Clearly that was the case with most of the kittypets we've seen in the series. Just because they went to the cutter (the vet) doesn't always mean they can no longer have children and become lazy.

With Millie, I guess I can sort of understand, because if I recall correctly, her owner was old? Maybe she forgot to do it, or figured it just wasn't important.
idk with Graystripe though. It's probably a retcon. Or the authors forgot.

I guess I just expected most kittypets to be neutered/spayed because where I live there aren't many who aren't.

Not everyone gets their cats fixed, even though they should. Here are some reasons I've heard
1. The owner may want their cat to experience motherhood at least once. 2. They believe their cat will change after getting fixed 3. They don't realize the importance of it, so they just don't do it 4. It was an accident 5. The cat is a farm cat or is feral, so the owner is unable to catch it and bring it to the vet 6. Money reasons

Throughout The Prophecies Begin, few kittypets were recurring characters. We only have Smudge and Princess. Smudge was neutered, Princess was not. For very minor characters, we have Henry, who was neutered before Into the Wild. But there's also Smudge and Firestar's kittypet friends. Were they neutered? Probably not, because Firestar only thought about Henry, who wasn't even his friend. Maybe the majority of his friends were kittens, but surely he had a few older friends or at least met some. And obviously, there's Jake, Nutmeg, and Princess' mate. (although we didn't know about them, logically we knew they existed) I don't think every kittypet was neutered. (as the Clans seem to think) I think it's just exaggerated a bit to discourage Clan cats from becoming kittypets.

I think in the early times of the Clans spaying and neutering kittypets was a more common thing in the area because there seemed like there was more cats in the area at the time and the twolegs decided to be more careful about spaying and neutering their pets. I think the Clan cats used this as a reason to convince kittypets (who were on the fence about joining) a reason to join. I think after SkyClan was forced to leave and less cats lived near/ came near the twolegplace the twolegs didn't feel the urgency to fix their pets as there was before so while some owners did fix their cats others didn't.
As for other places where kittypets have been seen, things that Twister mentioned may very well be the reason for it.
Actually I noticed there hasn't really been a spayed kittypet mentioned. There hasn't been a barren she-cat before (spayed or otherwise) I wouldn't be surprised if the twolegs don't feel a need to fix she-cats, just toms.

Most of the people I know that have cats don't spray/neuter them. They either don't live around a lot of cats and didn't have the need or just sell the kittens.

Unspayed female cats can be really annoying when they go into heat. Some even like to pee everywhere, like males do. But that's just my guess of why your mom did it. Or maybe she just didn't want to take a risk of her getting pregnant?

Maybe the two legs that captured Greystripe wanted to get the "ok" from the adopters. Millie's twolegs might not have believed in doing such a thing...maybe they wanted kittens themselves. That's where Greystripe comes in, to make kittens that the twolegs can sell later. Or, Greystripe could have even been several days until surgery before Millie's twolegs requested a full grown male cat because they actually wanted kittens. I doubt I'm correct but it's a weird thing to think about...
