Bramblestar noticed Leafpool was fat, but he just assumed it was due to prey. I guess the other cats just didn't think anything of it. It's possible for a cat to be pregnant but not have a very big tummy, so the others could just think they are gaining weight from eating too much.

I thought RiverClan cats were always described as a bit on the plump side though, because they mostly ate fish, so their fur was pretty sleek too, from fish oil and always swimming in the lake?

the clans just seem to take everything at face value. if someone is a medicine cat and they are gaining weight they think "oh they are getting fat. they aren't pregnant, they don't have a mate and aren't allowed to have a mate"
