I've thought about this too. All of the cats should have ticks, except maybe WindClan. I don't think ticks live in the grass, but I'm not sure. But all of the cats who live under trees should get ticks from time to time. It's weird how it's never mentioned though, same with fleas. They're hardly mentioned but the cats should be crawling with them.

Maybe the writer's logic is that those who have lots of leisure get ticks

I think all cats do it, but it just isn't mentioned much. Leafpool was mentioned to clean her pelt from ticks in Dawn or something. On the other hand, even though Elders can technically do it themselves, Apprentices probably do it for them out of respect. From what I could tell in the series, Elders weren't just old people who couldn't take care of themselves- they were/are treated with a lot of respect and were definitely high up on the clan hierarchy, maybe even higher than leaders- who don't have any "servants", so to say at all. Leaders would turn to them for advice, since often Leaders were younger than the elders, and so they would know more about a certain problem.
Another thing could also be that, as cats (and people) get older, they tend to have trouble with memory at times, or maybe forget to do basic tasks like clean themselves or clean the area around them. I noticed that as my cat got older, she started to groom herself less often and a year before she died, she stopped burying her litter. So I think it's partly out of respect and want to spoil them, but also cause some of them forget/or don't cate to take care of themselves if they're pretty old.
But it could be anything. It's often a punishment for apprentices, so it's mentioned a lot because of that cause apprentices getting punished is pretty common. There are several reasons why elders are most mentioned to have ticks.

They're hardly mentioned but the cats should be crawling with them. Grass? Are you kidding. Long grass is a breeding ground for ticks. You can take a dog for a walk in a field and get covered in 30 of them. I know. Personal experience.

I think it was mention once in a while that a cat had a flea or tick on them but then they removed it. Mouse bile (yuck) is pretty much a flea collar on a cat

I always thought that the Elder's just couldn't groom themselves as thoroughly as they could when they were younger so they needed apprentices to pick the tick off of them.
I just figured that everyone got ticks they were able to reach there ticks and get rid of them without problem.

there is a scene where leafpool grooms herself for ticks in dawn. I think ticks on elders is due to the fact they can't groom themselves well anymore. I believe it was in Dawn.

As Holly mentioned, Leafpaw(pool) was mentioned to have a tick at one point and Yellowfang has fleas in StarClan.
I think the main reason why it is never really mentioned is because it is not a major issue for able bodied cats. Elders can't twist around and reach all of the normal spots.
