In Power of Three, it always bothered me that when Graystripe returned, he simply accepted that Brambleclaw was deputy and everyone thought that he was dead. In my opinion, I think that it would have made Power of Three and Omen if The stars more interesting if the return of Graystripe had caused a power struggle in Thunderclan between him and Brambleclaw. Firestar would have been caught in a difficult situation. What could have happened if this did happen is that the power struggle would have led to the battle in Eclipse rather than Onestar having a grudge.
Another missed opportunity was Hollyleaf. In my opinion, Hollyleaf’s character arc should have ended in suicide because she couldn’t handle the truth of her birth, killing Ashfur, revealing the truth, as well as not being a member of the three. She should have actually forced Leafpool to eat the death berries before committing suicide herself. It would have made Hollyleaf seem more like a tragic figure. And it would have made her a potential suspect in possessing Bramblestar. It is unfortunate this did not occur.

Ravenpaw staying in Thunderclan. With this they could give him a lot of character development with how he turns into a good warrior and it would also be interesting to see what happens with Tigerclaw.
The Windclan civil war lasting longer, it also should’ve been from Windclan’s perspective. And if it was no one in the clan knew for sure that Onewhisker was chosen as deputy so it would cause more tension for the reader.

I wish WindClan brought back tunnelers especially since they live in a territory that already has pre-made tunnels. They could extend it further into enemy territory, use it for hunting during leafbare and this could bring up an interesting debate amongst the Clans on if WindClan tunnels outside their territory belong to them.

I think a power struggle between the two would have been interesting, but I never thought Graystripe should have been deputy in the first place.
I probably being biased since I love Hollyleaf, but I’m glad that she died in TLH. Her survival shows that you can come back from a mental breakdown, and I’m glad she had a warriors death.

I think the biggest things I'd change were leaderships/deputies, deaths and ships. Some quick ones off the top of my head: Brackenfur would have been deputy over Bramble and Gray. Beetlewhisker would have lived and become deputy in RiverClan Ivypool becomes the next deputy after the possessed arc is over. Either Mudclaw or Deadfoot would have made it to leadership. Tigertwo never becomes a thing period, Snowbird, Cloverfoot, or Dawnpelt, thanks.
I would have rather Sandstorm get with Dustpelt, instead of completely erasing their friendship over the mc needs to get a girl tm. Meanwhile, Firestar would have gotten with Cinderpelt, since imo Spottedleaf being with him is still weird. I'd rather Cinder follow her dreams as a warrior than being pushed to meddie over physical disabilities. Ferncloud can become Yellowfang's apprentice after. Gray x Silver can still happen but more screentime, and Silver lives, but moves to ThunderClan. Some other ideas that come to mind are Nightcloud x Mudclaw being Breezepelt's after Crow x Night. A late romance would have been nice. I feel like Storm x Squirrel would have been interesting, better Storm stays in TC with her than stay in a cult with Stockholm syndrome with Brook. I feel like he would have treated her better than Bramble or Ash. Toadstep getting with Blossomfall over Thornclaw would be preferable too. Tiger x Dove stays broken up after the first time, Foxleap lives and they become a thing instead.
I feel like Darktail being Onewhisker's son can still work as a plot even if he's not leader. Tigerheart dies in Dawnpelt's place, while she becomes deputy.

(Responding to OP on Hollyleaf's ending. TW: Suicide)
I always read Hollyleaf's story as ending in suicide, just a failed attempt because putting straight up suicide into a series aimed at 9 year olds is yikes. The dialogue in that scene reads very similar to trying to talk someone down from jumping off a building. Even if she was just running away and not trying to kill herself, I feel like that's less a reflection on the character/story, and more a reflection on the intended audience and what's considered mature content in our culture. To me it stands out as an obvious metaphor for something the authors are unable to explore directly.
As for my thoughts on your actual question, I feel like pretty much every single thing introduced in OotS fits the "missed opportunity" description, but I'm going to go with Rock's background. It's mentioned that the mountains are the ancestral homeland of basically all the cats we've met so far, with Rock mentioning his own life there, and I feel like this would have been good stuff to explore in books 3 or 4 instead of having Jayfeather scream into the void for 2/3 of the series.

Ivypool being a villain. I think it would have made OOTS more interesting rather than just bringing back Tigerstar and Brokenstar. She could also form an "evil Three" with Breezepelt and Redwillow and give Lion, Jay, and Dove a direct enemy to face.

More development for Fallen Leaves x Hollyleaf. It had so much potential and it would’ve made Hollyleafs decision to return to TC a lot harder, and more interesting for her novella.

Breezepelts redemption felt half hearted, in OOtS, he had malicious intentions and was a spiteful prick, but in CT, he suddenly felt feelings of guilt, which weren’t shown at all throughout OOtS. If he had a POV in OOtS it would probably be easier to sympathise with him and would give an insight to the dysfunctional family dynamic.
TNP being written from Brambleclaw and Leafpools perspectives. It should’ve alternated between Crowfeather, Squirrelflight, Mothwing/Stormfur/Feathertails and Tawnypelts perspectives.Then the last scene (Hawkfrost setting up Firestars death) would’ve been more suspenseful, because we wouldn’t already know for sure what Brambleclaw would do in that situation.

Rain's death. We could've had a full-scale civil war within the Kin where Darktail is killed (probably the way he was in the original) and Rain takes over. He could seek help from the clans for the fight and then turn on them after Darktail dies and become an even bigger villain. He'd probably want to drive the Clans out. Needletail could be caught between her love for Rain/a desire for power and her friends/doing the right thing.

Back on topic for real, I'd say show the start of Onestar's reign from Crowfeather's POV in Twilight. Have them show some cats thinking that Onestar acts like a house cat around Firestar, showing how he starts believing that he needs to be put their friendship on hold to put his Clan first. I think we were only told this in Cats of the Clans and not really shown.
