Mods had the power to appoint special titles, so it started with mods/admins appointing them to close friends. Eventually it transformed into anyone who asked got a special title from a mod, but that didn't go over well, so the mods' abilities to appoint titles was taken away. I don't know if it's been restored or not.

So, mods or admins gave people titles, mods may or may not be able to do that anymore. Admins still can, of course.
